Is there a js plugin to help cusotmers report errors?

I'm working on a client site (inherited) and we're getting a lot of error reports from customers. but they go through the process of emailing the cs rep and then me asking her for more information and then her emailing the customer and so forth.

I'm trying to find some type of plugin that could automatically add a form to certain pages (the ones behind login) where customers can quickly report a problem and I can have it attach pertinent information, like what page they were on when it happened.

I could hand-roll something, but I feel like I've seen something like this before, with a little help tab that pops out a form. But I cannot recall where and I can't seem to find the right terms to google. what I want is a "js plugin for customer error reporting" but I just get results for "js errors" or "custom error reporting" etc...

Does anyone know what (type of) plugin I'm talking about?

the site is running php/laravel if that helps at all.
