Multi Option selection in horizontal slider

Does anybody knows about a jquery plug in that lets me create a slider of options with images, option name, and also that let's me select multiple options, and if possible that let's me create a tooltip with an input to enter the amount that I want of that option?

Graphically what I mean is Having that slider I could specify the amount of products that I want of a single product and that also lets me select multiple options?

Also it's necessary for me to load the image to a html element, cause the images names are stored in db, and the view (html) is generated dinamically.

And also would like to know how could I send that data in a array to a php controller?
