Database design for different form requests

I need to handle different form requests for multiple departments needs. I was handling this with google AppScript which basically manage each form in one spreadsheet.

Actually i'm migrating forms to Laravel 5. My question in terms of design is if i should take this approach as example or if there is a different approach where i do not have to create a table for each form? Forms are emergent and i do not want the database to grow and lose control over it. Any suggestions?
